I, as a parent or legal guardian of a child participating in a Crossings Community Church^ event, do authorize Crossings Community Church to use any photograph/video taken at any event sponsored by Crossings Community Church in publications or other media material produced, used or contracted by Crossings Community Church including but not limited to: brochures, invitations, books, newspapers, magazines, television, websites, etc.

Any person desiring not to have their child’s photo taken or distributed must contact the staff member coordinating the event.
